Sometimes when processing background checks, the processing company is not able to fully confirm your identity. This error is known as a Name Mismatch. This error can occur for many reasons and is easy to fix. To address the error complete the following steps so we can get your background check back on track right away!
STEP 1: CONFIRM your information on the background check authorization is correct at portal.fieldworks.com by checking the following very carefully:
- Date of birth
- Social Security number
- Full legal name
If any of those are incorrect, MAKE CORRECTIONS and click SUBMIT. That will likely get things moving again.
STEP 2: If there were NO ERRORS, we will need to submit your IDs (both Social Security card and state-issued photo ID - NO OTHER IDENTITY DOCUMENTS WILL WORK) to our background check company for verification. Click the button below to submit photos of your IDs.

I already submitted my Background Check. Why did this happen?
Your background check has been submitted, but the date of birth, Social Security number, and/or legal name do not match. This is frequently caused by typos, hyphenated names, and Jr/Sr situations.
I was going to bring my birth certificate on my first day. Will that work?
The birth certificate is acceptable for your I-9 once your background check is approved and we get to onboarding. We have lots of flexibility for I-9s, but for this process with the background check company, only a state-issued photo ID and Social Security card will work.
