Thank you for canvassing for FieldWorks. We appreciate the time, energy, hard work, and focus you put in on this campaign. The project you have been working on is ending as of TUESDAY, NOVEMBER 4. The last day of shifts in your office will be Tuesday, November 4. Please see some important information below.
UBER VOUCHERS
- Make sure that you save enough of your Uber Vouchers to travel from the travel team hotel to the airport and from your arrival airport to your final destination after you land.
- NO additional Uber Vouchers will be issued.
TRAVEL HOME
- The travel team will be booking your flight home from Norfolk to the airport you departed from.
- If you need to change the airport you're returning to, you must let the travel team know by texting the travel hotline at 771-223-9009 no later than 5pm on Friday, October 31.
HOTEL CHECKOUT
- Your reservation at the travel team hotel ends at 11am on Wednesday, November 5.
- Regardless of the time of your flight, you must leave the travel team hotel by 11am.
TRAVEL HOTLINE
- After your shift on November 4, the office staff line will not be available to contact.
- If you have issues related to travel, reach out to the Travel Team at 771-223-9009.
TERMINATION
- There will be no shifts in your office after Tuesday, November 4.
- Your termination date will be November 4.
FINAL PAY
- Pay for shifts worked October 26-November 1 will be paid on Friday, November 7.
- Pay for shifts worked November 2-4 will be paid on Friday, November 14.
- All outstanding shifts and bonuses will be included in the final pay.
SEPARATION NOTICE
- You will receive a formal Termination/Separation Notice on November 5
- This notice will contain similar information to this site regarding future payroll issues, unemployment information, and other details.
FUTURE CONTACT
Your office will be closed after shifts on November 4 and you will need to reach out to our National Office regarding all matters including payroll. You will be unable to reach anyone at the local office in person or via phone or text. The National Office will be able to assist you with any issues you have. UNEMPLOYMENT
When filing for unemployment it is critical that you use the official FieldWorks filing information including company name and address. You cannot use the local address of the office you worked in. If you do not use the correct information, your claim may be delayed or denied. Our unemployment claims administrator is: ADP Unemployment Services/Corporate Cost Control.

WAGE STATEMENTS & TAX FORMS
You can access all of your wage statements and past tax forms online at http://my.adp.com. You can setup your account on a web browser or by downloading the ADP Mobile App. Instructions for access are at Create an ADP Mobile Account
A W-2 form for this year will be postmarked by January 31 of next year. It will be mailed to the address which appears on this form. W2 forms are also available online via the ADP app.
MEDICAL INSURANCE COBRA
If you were enrolled in medical, vision, or dental insurance your coverage ends as of the date of your separation. You will receive paperwork in the mail in the next few weeks on how to extend that coverage. Our COBRA administrator is TASC. You may reach TASC with follow-up questions at (800) 422-4661.
AGREEMENTS & CONFIDENTIALITY
As reminder even after your employment with FieldWorks has ended you are still bound by the staff and office policies you signed, including the confidentiality sections of the staff policies, which also includes the use of social media and talking to the press.
